• Cash memos from hospitals/chemists, supported by proper prescriptions.
• Receipts and pathological test reports from pathologists, supported by a note from the attending medical practitioner/surgeon recommending such tests.
• Surgeon’s certificate detailing the operation performed, along with the surgeon’s bill and receipt.
• Attending doctor’s/consultant’s/specialist’s/anesthetist’s bill, receipt, and diagnosis certificate.

For post-hospitalization, documents must be submitted within 7 days of the treatment date. After verification, the company will settle the claim.
