  2. One has to complete the pre-authorization form in the first place that is available at the hospital’s insurance desk. Show your Bajaj Allianz Health Insurance Membership Card as well.
  3. The form should be sent via email or fax to the company.
  4. An approval letter will be sent from the claim management of the Bajaj Allianz General Insurance (that means your claim is accepted).
  5. Now, the company or TPA will directly deal with the hospital bills.

Reimbursement Claim

  1. Download the claim form from the official website of the company or can take it from the nearby branch of the company.
  2. Submit the form along with the required documents such as discharge summary, prescription, etc to the company or TPA.
  3. Once the company receives the claim form, its team will analyze the same.
