  • Change of Gender Treatments
  • Treatment of obesity, dental problems, mental disorder, genetic disorders, abnormalities, congenital illness
  • Cosmetic or Plastic Surgeries
  • Injuries occurred by participating in hazardous or adventurous activities

How To Register A Claim For Star Super Surplus Health Plan?

Star Health claims to offer a hassle-free claim settlement process to all its customers. Read along to find out the two ways through which you can file a claim as per your convenience:

Cashless Claims

One can avail cashless claim settlement if they are opting to get treated in a network hospital of the company. Below is the detailed process of how you can avail cashless facility:

  • First thing first, be it planned or unplanned hospitalization, inform the insurance desk at the network hospital, and show your Star Health ID card for identification.
  • Download and submit the pre-authorization form.
  • The hospital authorities will verify your identity and submit a duly filled pre-authorization form with Star Health.
  • Once submitted, your documents will then be verified. If the need arises, the company may send out a form for further verification.
  • If approved, the company will directly settle all the amounts with the hospital.

Documents required for submission of cashless claim:

  • Health card
  • Doctor's consultation papers
  • Discharge summary
  • Investigation reports
  • Pharmacy invoices supported by respective prescriptions
  • In cases of accidents, Medico Legal Certificate (MLC) and/or FIR
  • KYC documents of the insured if the claimed amount exceed Rs. 1,00,000/-

Reimbursement Claims

In case one could not find a network hospital at the time of medical emergency, they can avail the treatment in any hospital and can later file a reimbursement claim. Below is the detailed process of how you can avail reimbursement:

  • All claims need to be intimated within 24 hours of hospitalization. Treatment can be taken at a non-network or network hospital of the company.
  • Avail treatment, settle all the bills, and file a claim for reimbursement.
  • Submit all the required documents like hospital bills, pharmacy bills, together with all the original documents of treatment and the claim form to the company within 15 days of the discharge.
  • After successful verification, the company will process the claim, and you will receive the amount in your registered bank account.

Documents Required To File A Reimbursement Claim:

  • Duly completed claim form including the present address, Contact number, and E-mail ID.
  • Original bills, receipts, and discharge certificate/card from the hospital.
  • Original bills from chemists supported by proper Original prescription.
  • Receipt and investigation test reports from a pathologist.
  • Surgeon's bill and receipt, if any.
  • Self-declaration/MLC/FIR in case of accident cases.
  • Treating doctor's certificate.
  • NEFT and KYC details.
